Freshmen applicants must be a Senior High School graduate of any institutions (local or foreign)
accredited by the Department of Education (DepEd).

– One (1) ID picture (1×1, latest)
– Form 138
– Certificate of Good Moral Character
– Birth Certificate from NSO (photocopy)

FOR TRANSFEREES

– Entrance examination (Waived for SY 2020-2021 due to COVID-19)
– One (1) ID picture (1×1, latest)
– Honorable Dismissal
– Scholastic Records / Copy of Grades
– Certificate of Good Moral Character
– Clearance for Admission from the Registrar’s Office
